When building your new consultation form, click Add question at the bottom and select your answer type.
Text - for when you want a written text answer to a question ie Tell us what you'd like to achieve today
Fill in the question box as you want the client to read the question ie Tell us what you'd like to achieve today
(Optional) You can fill in more info in the additional text box, maybe giving an example of what answer you'd like. You can make the question mandatory, or set character count restrictions
Date - for when you want a date format answer to a question ie What was the start date of your last period

Multiple choice - for when you want clients to choose one or more answers from a selection ie What magazines would you like on offer at your appointment

Decide whether you want clients to be able to select more than one answer by ticking the multiselect box.
In the choice box below, type out each answer, press Enter, repeat until all choices are listed. You can remove options by clicking the x
Number - for when you want a numeric answer to a question ie How many weeks has it been since you last box dyed your hair

Client Details - for when you want to update the client's profile directly with their contact details

Tick the boxes you want clients to answer - if selected, clients must fill in all fields to continue if the question is mandatory. The four options for SMS and email are for your clients marketing preferences. When a client fills in this question, it will directly update their client profile and overwrite their previous info.
