Go to Consultations on the left -> Click the form you want to edit

Click Edit form in the top right

If you want your clients to fill in this form once and then never again, leave the expiry unchecked. This means your clients will be asked to fill it in the first time a service triggers it. You can also choose that this is only for new clients to the salon below.

If you want a client to fill it in every now and again, check the expiry box and define the expiry time. For example, 6 months if your insurance requires a patch test every 6 months.
